Easy Steps Ahead to Make an Appointment. Get Ready, Set and Go!
Step 1
Check Insurance
Confirm we’re in-network for your plan (or ask about out-of-network benefits). If we do not take your insurance, ask us about self-pay option.
Step 2
Referral Sent
Ask your provider to fax referral + notes/records to 941-485-2150.
Step 3
Pre-Register
After we receive the referral and records, we will contact you with a secure online pre-registration form. Please check your emails or cell for a message from us or Phreesia. The link is only good for 24 hours. Please complete everything. If you need to stop, it will save what you completed and you will scroll through what has been completed to finish. Please call us if the link expires so we may resend it to you. Office 941-485-2220.
Step 4
Schedule Visit
After pre-registration is complete, we’ll call to schedule your appointment.
Step 5
Try for Sooner
Join our cancelation list—openings happen and we’ll try to move you up.
Big things to know:
Big thing one: 20 minutes early every appointment every time…There is always something to sign and confirm at check-in even if you checked in online. We need to get you back to the room and ready to see the doctor and that takes time. Your appointment time is with Dr. Lifton and that 20 minutes gives us time to update information, take vitals, and welcome you to your visit and ask a few questions. Please do not ask for an exception to this.
Big thing two: Insurance is tricky, and we do our best but we do focus on your healthcare and ask that you know your insurance so there are no surprises. Insurance will never guarantee coverage until they get a claim and review it. So, please be aware of that nuance. Also, check for out-of-network benefits, in-network benefits, deductibles, copays for specialists and co-insurance.
Why Are We Referral Based?
To ensure Dr. Lifton can provide the precise, comprehensive care you deserve, we operate on a referral-based model. This is not insurance related.
Prior to starting the pre-registration process and scheduling your first appointment, we must first receive a formal referral and accompanying note from your referring physician.
This allows us to dedicate the necessary time to thoroughly review your full medical history and prepare for your specific neurological needs. Please ask your primary care provider to fax or send this documentation to our office.
Our FAX number is 941-485-2150.
Call our office at 941-485-2220 with any questions. If we are with other patients, please leave a detailed message on the voicemail system so we can get started after we receive your message.
If you request an appointment, please follow up with a phone call to our office 941-485-2220. The steps above will still be needed.
