2) Send Your Records
Fax your release to your physicians and send records to us via fax 941-485-2150 Attention Chart Review.
3) Physician Review (Hourly)
The doctor reviews the materials you provided and identifies key findings, gaps, and priorities.
4) Written Summary
You receive a written summary and next-step suggestions to discuss with your treating clinician.
What you’ll need
- Your completed Release of Information form
- A list of your doctors/facilities (name, phone, fax if available)
- Key items such as consult notes, imaging reports, labs, and procedure reports
- Your specific question(s) for the review (1–3 is ideal)
Tip: The more complete the records, the more useful the written summary will be.
